This form is for couples who plan to hold their wedding at a location other than a Bethany Church campus but would like to request a Bethany pastor to officiate their ceremony.

Please complete the form in its entirety. A member of our team will contact you once your request has been reviewed.

Please review the following guidelines carefully before submitting your request:

All weddings must align with Bethany Church’s qualifications and Statement of Faith regarding marriage.
Weddings cannot be scheduled on Sunday mornings.
Requests must be submitted at least ninety (90) days prior to the desired wedding date.

Premarital Requirements

Bethany Church requires all couples to complete premarital counseling prior to the their wedding ceremony. Once your information is finalized, a member of our team will contact you to coordinate your premarital counseling sessions and the SYMBIS assessment, a tool used in counseling based on responses provided by both individuals.

Please note the following fees for these items:
SYMBIS Assessment: $35 given to Bethany Church
Premarital Counseling: $150 given directly to the pastor/counselor

Which Bethany Church pastor would you like to officiate your wedding? *

Please note: An honorarium of $350-$500 is required and payment for officiation must be made by cash or check, payable to the officiating pastor, and delivered or mailed to Bethany Church office no later than two (2) weeks prior to the ceremony date. Mail payment to:

I have read Bethany Church's wedding contract, guidelines and associated fees.

I understand that the following payments are required:

SYMBIS: Paid directly to Bethany Church prior to assessment being provided.
Premarital Counseling Fee: Check payable to the pastor/counselor, mailed to Bethany Church one (1) week prior to the first counseling session.
Officiating Pastor Honorarium: Check payable to the officiating pastor, mailed to Bethany Church two (2) weeks prior to the ceremony.

I understand that any changes to this contract are subject to pastoral discretion and the availability of qualified personnel.
